1. Cash-Based Services

Smart Functional Fitness LLC is a cash-based physical therapy and wellness practice.

We do not bill insurance.

We are not contracted with Medicare, Medicaid, or private insurance providers.

Payment is due in full at the time of purchase.

A superbill may be provided upon request for possible out-of-network reimbursement.

Insurance reimbursement is not guaranteed.

You are fully responsible for all fees.

2. No Medical Guarantee

Services provided include physical therapy, therapeutic yoga, strength training, personal training, and wellness education.

Results are not guaranteed.
Individual outcomes vary.

Participation is voluntary and you assume all risks associated with physical activity.

3. Refund Policy

All sales are final unless otherwise stated in writing.

Session packages are non-refundable.

Missed sessions without 24-hour notice may be forfeited.

Digital programs are non-refundable once accessed.

4. Cancellation Policy

A minimum of 24 hours’ notice is required to reschedule or cancel an appointment.

Late cancellations or no-shows may result in loss of session credit or full session charge.

5. Chargebacks & Payment Disputes

By completing this purchase, you agree not to initiate chargebacks or payment disputes without first contacting Smart Functional Fitness LLC to resolve the issue.

Unauthorized chargebacks may result in collections and termination of services.

6. Assumption of Risk & Release

By purchasing, you confirm that:

You are medically cleared to participate.

You voluntarily assume all risks related to exercise and physical activity.

You release Smart Functional Fitness LLC and its representatives from liability for injury, loss, or damages to the fullest extent permitted by law.
